Title: CERTIFICATIONS REQUIRED WITH DELIVERY OF MATERIAL (FISCPH)

All Certifications are required to be delivered with the material. Payment on material for which certifications are not received will be withheld. SHELF LIFE ITEMS (FISCPH) Supplier shall furnish the date of manufacture and shelf life on shipping and/or other documents which accompany all deliveries on shelf-life items. MERCURY EXCLUSION FOR MERCURY FREE PROCUREMENT (FISCPH)JUN99 1. Mercury or mercury compounds shall not be intentionally added or come in direct contact with hardware or supplies furnished under this contract. Supplier shall certify that all items furnished under this contract are free of mercury compounds. HAZARDOUS MATERIAL NOTIFICATION (FISCPH) Supplier is responsible to advise buyer for instructions within 5 days of receiving award whether or not product being supplied is a flammable liquid or solid as defined as those fluids, including those liquids contained in aerosol containers, with a closed cup flash point below 100 degrees Fahrenheit. Flammable gasses and solids are defined as those that are listed in NFPA standard 325, Fire Hazard Properties of Flammable Liquids, Gasses, and Volatile. Supplier is also responsible to fax MSDS to Buyer within 5 days of receiving award. Supplier is responsible to advise Buyer for instructions regarding refrigeration and/or other environmentally controlled handling and storage requirement. MATERIAL SAFETY DATA SHEET (MSDS) For all products that require a Material Safety Data Sheet, (MSDS), a copy must be provided with each shipment.
